| Job Description:

Seeking experienced RN Case Manager who is caring and compassionate to help care for our clients. The RN Case Manager will direct nursing care for hospice clients of all ages in their place of residence, coordinate care with hospice team, family and paitent. The Case Manager assumes the responsibility for the coordination of care. |

Job Requirements:
|
Education: Graduate of accredited school of nursing.
Licensure: Current Texas State RN license, Cuirrent Texas Drivers License and Current CPR Certification
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Must have reliable transportation
2 years experience as an RN in a clinical care setting. Hospice experience is preferred.
